Diaphragmatic function assessed during an SBT trial using m-mode ultrasonography and MIP: a pilot study

چکیده
Introduction Acute respiratory failure (ARF) is characterised by a discrepancy between load imposed on respiratory muscles and their capacity. The evaluation of inspiratory muscles performance is based mainly on Maximum Inspiratory Pressure (MIP) calculation. However, MIP is determined by the contraction of all the inspiratory muscles and doesn’t give information about diaphragmatic function. Recently the Diaphragmatic Displacement (DD) measured by ultrasonography has been introduced in the clinical practice to evaluate diaphragmatic function.

Can diaphragmatic ultrasonography performed during the T-tube trial predict weaning failure? The role of diaphragmatic rapid shallow breathing index
BACKGROUND The rapid shallow breathing index (RSBI), which is the ratio between respiratory rate (RR) and tidal volume (VT), is one of the most widely used indices to predict weaning outcome. Whereas the diaphragm plays a fundamental role in generating VT, in the case of diaphragmatic dysfunction the inspiratory accessory muscles may contribute. If this occurs during a weaning trial, delayed we...
متن کاملPoint-of-care ultrasonography and liberation from mechanical ventilation
Safe liberation from mechanical ventilation remains a challenge for the critical care physician. Undue prolongation of mechanical ventilation or extubation failure followed by re-intubation is associated with negative outcome. Recent clinical practice guidelines suggest using pressure augmentation during a spontaneous breathing trial (SBT), which would result in more successful SBTs and higher ...
متن کاملDiaphragmatic amplitude and accessory inspiratory muscle activity in nasal and mouth-breathing adults: a cross-sectional study.
The purpose of this study was to evaluate the electromyographic activity of the accessory inspiratory muscles and the diaphragmatic amplitude (DA) in nasal and mouth-breathing adults. The study evaluated 38 mouth-breathing (MB group) and 38 nasal-breathing (NB group) adults, from 18 to 30years old and both sexes. Surface electromyography (sEMG) was used to evaluate the amplitude and symmetry (P...
متن کاملUltrasonographic Diaphragmatic Motion Analysis and Its Correlation With Pulmonary Function in Hemiplegic Stroke Patients
OBJECTIVE To evaluate diaphragmatic motion via M-mode ultrasonography and to correlate it with pulmonary function in stroke patients. METHODS This was a preliminary study comprised of ten stroke patients and sixteen healthy volunteers. The M-mode ultrasonographic probe was positioned in the subcostal anterior region of the abdomen for transverse scanning of the diaphragm during quiet breathin...
متن کاملThe predictive value of serial changes in diaphragm function during the spontaneous breathing trial for weaning outcome: a study protocol
INTRODUCTION There is a variety of tools being used in clinical practice for the prediction of weaning success from mechanical ventilation. However, their diagnostic performances are less than satisfactory. The purpose of this study is to investigate the value of serial changes in diaphragm function measured by ultrasound during the spontaneous breathing trial (SBT) as a weaning predictor. ME...
